What can you expect?

At Community Care Collaborative your medical team isn’t only made up of GPs. Each healthcare professional has extensive knowledge, experience, and training. This includes our Advanced Clinical Practitioners, Practice Nurses, Mental Health Practitioners, Social Workers, Physiotherapists, GPs, and more.

Who am I going to see?

Advanced Clinical Practitioner (ACP)

As an ACP, I see patients with a variety of health conditions. I am a Registered Nurse or paramedic who has completed a masters degree and more specialist training and can prescribe medication and treat complex illnesses.

What can I treat?

  • Allergies
  • Flu-like symptoms such as fever, vomiting
  • General ongoing health concerns
  • Genital complaints such as urinary tract infections and sexual health concerns
  • Headaches and mild head injuries
  • Unwell children
  • Limb swelling
  • Abdominal (tummy) complaints such as pain, diarrhoea, constipation or piles
  • Short-term back pain and joint sprains
  • Short-term flare up or worsening of an existing condition
  • Women’s health

General Practitioner (GP)

As a GP, I am highly trained in assessing patient’s needs . At CCC GPs will focus on assessing and treating those patients with the most complex or serious medical problems. In doing this we will be able to ensure that those people with the greatest need will always get to see a GP when they need to and that the GP will have the time to spend ensuring that we provide the best care that we can.

  • Individuals with complex or serious care needs
  • Multiple health problems
  • Cancer and terminal illnesses
  • Homeless and rough sleepers
  • Specialities musculoskeletal problems
  • Dermatology (skin problems)
  • Minor surgery
  • Women’s health and menopause

Practice Nurse

As a Practice Nurse, I am a familiar face in the practice who supports family health matters (such as jabs and travel advice), patients with long-term health conditions that require managing and minor illnesses.

What can I treat?

  • Management of chronic diseases such as high blood pressure, asthma, COPD, diabetes
  • Immunisations (jabs) for adults and children
  • Smears, contraception

Mental Health Practitioner

As a Mental Health Practitioner, I have had extensive training in treating and managing mental health conditions. I run clinics for different conditions such as depression or anxiety, and I also run group support sessions.

What can I treat?

  • Anxiety
  • Low mood
  • Long term mental health conditions e.g. bipolar disorder
  • Eating disorders
  • Obsessive-compulsive disorder
  • Post-traumatic stress disorder
  • Addiction(s)
  • Perinatal and post-natal mental health

Minor Illness Nurses

What can I treat?

As a Minor Illness Nurse I have done specialist training and I am qualified to prescribe .

  • Acute exacerbation or deterioration of a chronic disease
  • Bites
  • Minor burns
  • Coughs & colds
  • Ear infections
  • Skin infection
  • Wound care
